How they differ
Tirzepatide is a single peptide engineered to bind both GIP and GLP-1 receptors. CagriSema is a co-formulation of two distinct molecules: cagrilintide, a long-acting amylin analog, and semaglutide, a GLP-1 agonist.
The mechanistic difference is the amylin pathway. Amylin is co-secreted with insulin and acts on satiety through the area postrema — a route independent of the incretin system entirely. So CagriSema combines two unrelated satiety mechanisms, where tirzepatide combines two incretin ones.
Formulation matters for handling: a co-formulation means two active compounds in one vial, each with its own stability profile.
