Overview

Cagrilintide (NN9838) is a long-acting acylated analog of human amylin (islet amyloid polypeptide, IAPP) developed by Novo Nordisk. Amylin is a 37-amino-acid peptide co-secreted with insulin from pancreatic beta cells after meals. Native amylin has a very short half-life (~15 minutes), whereas cagrilintide's acylation with a C18 fatty di-acid enables albumin binding and extends its half-life to support once-weekly research dosing protocols. It is being studied both alone and in combination with semaglutide (CagriSema) in multiple clinical trials.

Mechanism of Action

Cagrilintide binds to amylin receptors (AMY1, AMY2, AMY3 — heterodimers of the calcitonin receptor with RAMPs 1-3) in the area postrema and hypothalamus. Activation of these receptors promotes satiety signalling, delays gastric emptying, and suppresses postprandial glucagon secretion. Unlike GLP-1 agonists, amylin receptor activation engages distinct neuronal pathways including histaminergic and dopaminergic circuits in the lateral dorsal tegmental area. The complementary mechanisms explain the additive effects observed when combined with GLP-1 receptor agonists.
